  • 100% State-Funded Services: Fully supported by Medi-Cal’s ECM benefit, offering zero-cost care for eligible individuals.
  • Community-Based Coordination: Collaborating with CBOs to train staff in identifying and engaging high-needs individuals for seamless service access.

Housing Support
Assistance with securing stable housing, including deposit support and coordination with housing programs
Behavioral and Mental Health
Integrated behavioral health services, including LCSW support and psychiatric coordination, to address SMI and reduce recidivism.
Transportation
Coordination of care-related travel, including bus passes and other transportation support.
Chronic Care Management
Medication management, chronic condition education, and virtual urgent care to reduce ED visits and improve health outcomes.
Social Support Services
Assistance with job searches, financial planning, food resources, and ADLs to support long-term stability.
Meal Assistance
Access to nutritious meals through food programs, meal delivery services, and community partnerships to support overall health and well-being.
