Who Is Pair Team?

Pair Team is a leading provider of whole-person care. Our team of care managers, nurses, and mental health clinicians assist patients with social needs (such as finding housing, accessing food, or getting a job) as well as medical and mental health needs. Each patient is matched with a Lead Care Manager who identifies and helps with your care coordination.
