Maria’s journey
Maria went to a local community based organization looking for housing. After getting to know her, we learned she also needed access to food, medications, and no-cost health insurance.

Real lives, real care.

  • Week 1
Maria quickly enrolled online so we could help address her needs to find housing support.
  • Week 2
She worked with our clinical care teams to get additional support, like help connecting with a local pharmacy to refill her depression medication.
  • Week 3
In need of urgent care, Maria contacted us for same-day support and avoided an unnecessary emergency cost.
  • Week 4
She was connected with a PCP after a three year gap, as well as transportation assistance and appointment prep.
  • Months 2-4
With the guidance of Maria's PCP, we did the work to align on her care goals, progress, and support follow-ups.
  • Months 5-6
Within her first six months, Maria closed her gaps in housing, medication, scheduled and emergency doctor's appointments, and established a care plan for her future.

Whole person care for all.

When your basic needs aren’t met, it’s hard to stay healthy. Pair Team helps remove those barriers by connecting you to real care that supports your whole life — medical care, mental health, housing, food, and more. Our team is here to make sure you get the help you need, when you need it.
